We understand that having any type of healthcare procedure can be overwhelming. If you are scheduled for an Outpatient procedure at AnMed Health Cannon , providing the following information will help in making your experience as pleasant and prompt as possible.

Pre-Register and Save Time

If your physician has scheduled you for a procedure, please call 898-1242, Monday-Friday between 8:30am and 5:00pm to pre-register before your appointment. This will allow us to meet your insurance pre-certification requirements and also decrease your wait time.

Information Needed for Pre-Registration:

– Name, Address & Phone number
– Social Security Number
– Date of Birth
– Insurance Card information
– Employer Information
– Guarantor Information
– Date of Procedure
– Doctors Name
– Type of Procedure

Information needed when you arrive

Please make sure you bring the following to your procedure:

– Doctor’s order
– Insurance Information
– Photo ID
-Advance Directives (If you do not have a Health Care Power of Attorney or Living Will, please let Cannon Providers know.  They can provide you with 5 Wishes directives.  You may also find more information at the website: National Healthcare Day of Decision or by contacting your attorney.


Instructions the Day of Your Procedure

When you arrive for your appointment and you have already pre-registered, please sign in at our outpatient department (click here for outpatient parking information) and tell the receptionist that you have already pre-registered. We will then make a copy of your insurance card and have you to sign your consent. You will then be directed to the correct department. Our staff is required to collect any co-pays or deductibles at the time of registration.
